Membership Cancellation Request

If I reactivate my membership in the future, the rates in effect at that time will apply, and I cannot return to my old rate.

If I miss more than 60 calendar days, I will need to restart with the 4 session fundamentals for safety purposes.

A 30-day notice is required to cancel my membership, and this form serves as that notice. I will retain my membership services until the notice period ends.

If I do not submit this cancellation form, my membership will not be canceled, and I will continue to be charged. All payments made are non-refundable.

By submitting this form, I understand and consent to being charged for 30 days of membership per the cancellation notice.
